The 'KT' tax/price code is stamped only on the label side one.
Housed  within a front laminated,  tri-flipback  'Garrod & Lofthouse'   printed mono sleeve.
Released in UK 1964.
 
Complete with its original   rice-paper-lined ‘Use Emitex...’ inner sleeve.
Matrix numbers:  XEX 473-1N  &  XEX 474-1N, Stamper numbers: 1/P  &  1/O
